STEP – 1
SELECTING A PACKAGE
Family will need to select a care package from the option of short term care and long term care depending upon the scope of services required for their family member. There are three options to choose from ranging from low care to high care. These packages are called SEVA, SADHNA and SAMARPAN. Depending upon the amount of care required to the person, the choices can be made. The service user will receive care, facilities and services depending upon the package being selected.
STEP – 2
SERVICE AGREEMENT & DOCUMENTS CHECK
The primary guardian (a responsible family member / decision maker / blood relative of service user) will have to undergo a mutual service agreement which will make family understand of what to expect from KADJI and terms & conditions for residential care. This will be accompanied with documents submission. The documents which accomapny the admission forms would be: 1. Two photographs of service user, 2. Photo ID and address proof of service user, 3. Phot ID and address proof of primary guardian, 4. Laboratory investigations of the service users.
STEP – 3
SECURITY DEPOSITE AND PAYMENT
The security deposite is mandatory in case of long term care of service user. The security deposite should not be used as a payment of service user.The place of the service user will be confirmed after payment and security deposit.
STEP – 4
TRANSFER OF SERVICE USER TO CARE FACILITY
Transfering the service user at kadji from his/her home is always a sensitive situation. One must make sure that service user is briefed about where and why he/she is being taken. Kadji can assiste family in counselling process if required. The institute also have third party ambulance services which can be hired by family in case of prior request is made.
