VISIT OUR FACILITY

Visiting a care facility helps family to familiarise them selves to the people who are receiving services of kadji and also to people who provide their services. At kadji, family has multiple options for different location and room facilities. Visiting the place help them understand their need for their family members. Feel free to ask for “visitor co-ordinator” once you arrive at KADJI. They will help you introduce to the facility and services available.

SELECT THE LOCATION / ROOM / PACKAGE

Family will need to select a care package from the option of short term care and long term care depending upon the scope of services required for their family member. There are three options to choose from ranging from low care to high care. These packages varies based on which branch/centre one chooses for their family member. The cost of services will depend on what kind of facilities family is looking for.

SERVICE AGREEMENT / DOCUMENT SUBMISSION

The primary guardian (a responsible family member / decision maker / blood relative of service user) will have to undergo a mutual service agreement which will make family understand of what to expect from KADJI and terms & conditions for residential care. This will be accompanied with documents submission. The documents which accompany the admission forms would be: 1. Two photographs of service user, 2. Photo ID and address proof of service user, 3. Photo ID and address proof of primary guardian, 4. Laboratory investigations of the service users. The ID proof of both admitting person and a primary guardian will require to submit to the office.

PAYMENTS AND DEPOSITS

Payments can be made in Cheque, through bank transfer or in cash at the office. Family is requested to receive an invoice and a receipt of payment against every payment.

TRANSFER OF RESIDENT AT KADJI
Transfering the service user at kadji from his/her home is always a sensitive situation. One must make sure that service user is briefed about where and why he/she is being taken. Kadji can assiste family in counselling process if required. The institute also have third party ambulance services which can be hired by family in case of prior request is made.
RESIDENT ASSESSMENT
There is an intestine assessment process once resident is admitted. Various aspect of a resident ranging from their previous medical history, social background, physical status, mental status to their likes and dislikes are assessed and documented in a system. The same is reviewed periodically in the presence of a family member by a qualified medical practitioners.
CARE PLAN FORMATION AND EXECUTION
Care plan execution team comprises of Health officer, Nursing incharge, Personal care assistants, physiotherapists and physician. A resident is assigned his PCA within 4 hours of admission and she is explained of care plan duties.
